I consent to the use of personal data for the purpose of submitting an application for obtaining a marriage license in the state of Utah, for registration for a wedding ceremony conducted via video communication, for the delivery of a paper marriage certificate by mail, for interaction with the authorized body of the state of Utah on issues related to the preparation and registration of the ceremony and the delivery of the marriage certificate by mail, and other purposes directly related to the organization of the wedding, as well as for storing data on these results on electronic media.

This consent is granted by me for actions with respect to my personal data that are necessary to achieve the above purposes, including (without limitation) collection, systematization, accumulation, storage, clarification (updating, changing), use, transfer to third parties for actions related to information exchange, depersonalization, blocking of personal data, as well as any other actions provided for by the current legislation of the Russian Federation.

This consent is valid until the purposes of processing personal data are achieved or for the duration of information storage.
